To simplify it, we can say it's 0 to 100. 90 to 100 are absolutely confident in their gender. 0 to 10 actually have dysphoria. Everything in between are varying levels of it.
In the past these varying levels would deal with it in various less destructive ways, such as simply being an effeminate man or a tomboy.
Nowadays instead every kid who isn't a 100 is told their ARE transexuals and MUST take hormones, which further fuck up their stability.
Years later they have permanently fucked up hormone balance, possibly have undertaken irreversible surgery, are suffering from a million traumas and they kill themselves because they were never trans, they were just a tomboy who got fooled.
