Kiara's design is bad because she looks like an actual person cosplaying as an anime character. Have you seen her roommate? Her hairstyle is exactly the same, except for the dyed bits. The body type is the same, even the outfit doesn't look too outlandish compared to the other girls'. That's why many people feel some kind of uncanny valley effect when looking at her.

As far as I know they do now. Noel and Coco mentioned how they asked for bigger boobs on their characters, but older gens didn't and they auditioned for an already existing character.

depends on the generation.
suisei got full say since she just uses the same model she had when she was an indie (and had to fight cover for it), most gens can ask for changes for the characters, i think 0 and 1 gen didn't get any say and at the time were just given a ready made character.
