Essentially Polka struggles with Anxiety, and doesn't like putting herself in situations where she can't go home if she starts to feel panicked. The "rehab" refers to Flare helping Polka to try and work through the anxiety by doing small things like going out to eat or having a sleepover at Flares, to try and get her used to being in those situations with just the two of them, and working towards her being able to handle it with more people.
As for the Birthday Live, Flare also mentioned that she tried to reach out to people she wanted to sing with before but had not had much of a chance too, which is why many of the usual (Polka, Fubuki, Noel, Marine) are not participating. And fwiw I have no doubt Polka is pleased that she is, since Flare used to be really bad at reaching out to new people.
