Smoking is actually one of those touchy subjects for idols they aren't supposed to breach. There's been big controversies in the past when traditional idols have been caught smoking. So I don't think they'd ever talk about it on stream at least.

There was exactly one klicking, similar to an electric lighter, after one of her streams. Could be fucking anything. I don't believe smokers often go 12 hours without any, in particular if they experience stress. But believe what you will.

It depends. It gives them a harsher voice, which is good for some kinds of music, like blues. Cat Power is an example of this, she's smoked for a while and had a good bluesy voice.

This could also potentially mean that her willingness to interact with another holo is a good litmus for how likely they are to be a smoker.

My supervisor basically lives in the gym outside of work and does competitive lifting, and the only time he doesn't have chew in his mouth is when he's outside for a smoke break. Being physically active doesn't prevent you from being an idiot.
