If your neighbors find out they'll try to run you off, employers will shun you, public services workers-- the people responsible for paying for your treatment and potential neetbux-- may harass and ignore you; in a lot of ways it's better to just tell everyone you're dropping out of society to become a leech than openly admit you've got a problem and try to continue being a productive member of society. Things were changing in this regard when I lived there but Japan was, and still likely is, an awful place to be mentally ill.
