If your neighbors find out they'll try to run you off, employers will shun you, public services workers-- the people responsible for paying for your treatment and potential neetbux-- may harass and ignore you; in a lot of ways it's better to just tell everyone you're dropping out of society to become a leech than openly admit you've got a problem and try to continue being a productive member of society. Things were changing in this regard when I lived there but Japan was, and still likely is, an awful place to be mentally ill. Holomems with mental or emotional illnesses are incredibly lucky to have an employer like Cover.
