If you've been pretty much functional without any medication for ADHD, then you can say as such to a doctor and refuse. There's nothing wrong with that. At the same time, if medication does help, more than anything else that you've tried before, there's also nothing wrong with taking medication for ADHD, assuming the side-effects don't severely affect your quality-of-life. If they do, stop taking the pills and tell your doctor.
The important thing is 'mostly functional. For those who are not, the medication is obviously the difference between being able to do stuff for yourself and being an invalid, so it's not even a question.
