Other cases, an intersex person wants to go in the "other" direction than what their genitals are nudging them towards. Like if their external genitals are mostly female looking, but their internal ones are producing testosterone.
Or, sadly, as other people have said, there are occasional cases of trans people claiming to have intersex conditions that they do not actually have, because they see intersex conditions as "more valid" than being trans. This is unfortunate, since the community of those with typical intersex conditions is struggling a lot with awareness of their unique circumstances and experiences.
I don't know this particular streamer and their circumstances. Nor do I think any of their audience can possibly know the streamer well enough to make a judgement like that without access to their medical history. I'd be inclined to take them at their word for it, but also take advantage of the learning moment to seek out other stories of intersex people for comparison. It'd be unhealthy for you to base your entire understanding of intersex conditions and how society interacts with them based solely on "that vtuber I know!" when there are many distinct intersex conditions with different consequences.
