Now, before a campaign starts, usually GMs and players get a sense if it's going to be a high mortality campaign or not. Most aren't, but some games are specifically designed around you not getting TOO atached to your charcters. See Only War.
On the other end, some DMs will go out of their way to make sure even the dumbest decisions won't result in death, with them either secretly fudging rolls, or changing the narrative and saying someone got knocked out.
I don't like the later as completely removing death as a consequence removes a lot of tension- unless you're specifically going for a saturday morning cartoon feel.
