At some point, Gura legitimately had a health scare. She took the time off, went silent, and everyone both inside and outside the company just assumed she'd recover and be back to normal in a month or three. This isn't anything abnormal for a Hololive talent.
However, during this time a no activity, Gura realized she was making just as much money as when she was active. Or at least was making so much she was comfortably living and then some. Gura also hates the pressure of being the poster child and top talent numbers wise. So she simply decided she'd heavily slow down her output. Because why not? If any of us can live well off doing nothing, we would.
Management and Gura are not at an impasse: Cover doesn't want to lose a merch cash cow like Gura, and Gura doesn't want to lose her (mostly) passive income. So all Gura has to tell management is "Oh nyo, health issues!" and they begrudgingly accept giving her a light contract to keep her officially active.
