The reason Japan uses pick and cotton styled tools for ear cleaning is due to them having hard earwax that solidifies and has to be picked out
Whereas Euros have wet earwax that needs to be swabbed out or vacuumed like the Germans do with a hollow tube they light on fire to create a suction effect
It's actually very nice and I recommend getting it done, had a German masseuse that would do it for me and it was actually relaxing
