It's a fairly new convention. How much external marketing you do really makes a difference, but USA is a really, really big place.
Pretty much every single vtuber fan in germany would attend something like that since it's the only thing and they just need to catch a bus to go. Sometimes demographics and location really work in for favor like that.
Japan is the absolute best example, they can operate extremely mega niche maid cafes and one-off special cafes for c tier anime stuff and remain profitable.

My understanding is that this convention also operates like a networking event for marketing, even Virtual Market from vrchat is there
