basically because most other countries don't (or at least didn't decades ago) have fast food as just a meta thing everyone just accepts exists and is normal, in some places they make it significantly fancier. people there expect any restaurant you go to to be a sit-down place and to put at least some effort into the food because that's normally what it's like for everything else that isn't a street food cart.
in the US the meta is that everyone just-knows that of course, you can go to like 10+ places to get hot burgers (or maybe something else if you're near a taco bell or jack in the box etc) in 2 minutes basically any time of day that are cheap and never were expected to be top-tier gourmet material, but are good-enough to keep you going. you'd think in such a competitive environment mcdonald's would try harder to keep up with other burger places but it seems they for whatever reason don't - I think, because if you have a preferred place in your mind already you're just gonna go to that one most of the time regardless of what the other places are doing, for example I never eat burger king and would rather drive twice+ as far to anywhere else to get a burger besides there.
