I think for this scenes it was inspired by how he got the actress to portray her disgust by describing the scenario and then asking her how she would feel, and she said "disgusting" and he said that's it. The actress said he was a pretty weird guy, too. You have to understand the broader context though, with otaku no video, and the fan reactions and death threats for the Evangelion tv show. It makes perfect sense why he put this scene and other scenes in.
