You don't have to make it a profession to be "real," which is kind of an ironic term to use for vtubers.
IMHO to be a legitimate vtuber you just have to stream and use a dedicated avatar persona over other visual identity.
While I don't really care if people just use it as a temporary novelty while mostly streaming without avatar or with a facecam, that displays a lack of dedication to the persona.

I wanted to become an Idol, but after a bumpy ride I ended up failing and my only options were to either leave Japan and become a cashier in mcfastfood, or become a vtuber. It felt humiliating at first, but by now I got used to this more or less.
