What's going on is pretty simple to understand because the main disease isn't what's fucking her up, it's all the other common illnesses she accumulates because of it and how they take 10x longer to get better and thus leave 10x more damage. If you can understand the flu, you can see what's going on. Her lungs for example are scarred to hell and back (scarring never gets better) from having pneumonia, covid etc. multiple times and there's no need to hypothesize about that since she's had the illnesses on stream enough that it's guaranteed she has lung damage if they were literally the only times she had ever been sick.

If you do it into a microphone you can sound very loud while not straining your lungs, downside is that it's very high pitch. In recent times she's sang a little in her more normal voice range, look up "iron mouse shakira voice", it's pretty freaky.
What to me is most striking about her extremely poor health and being relatively close to death is that she doesn't sleep nearly enough to live normally. During subathons (and long streams) you can see she's regularly sleeping for only a couple hours before she's back online. Her life makes sense if you think of it as someone who's terminally ill with all the gifts, lavish spending, and events she does (including the ones that make loads of money), she's like Connor's make-a-wish milf who is trying to leave money for her family when she passes. She's like Walter White.
