Not just in terms of 'why is transgenderism a thing in a high fantasy setting in-universe' but also why, if you are pro-transgenderism, would you deliberately make an idealized fantasy setting in which transgender people need to exist (suggesting there are people with dysphoria or whatever in the setting) and still get misgendered/anti-transgender people still exist in-universe?
It doesn't seem to make any sense at all
