James Sunderland killed his wife because he could no longer cope with the loss of his personal freedom nor the stranger his wife had become. This caused him to have a psychotic break, and in this uniquely vulnerable state Silent Hill's paranormal aspect was able to start attacking him. He came to the town with his wife's corpse with the intention of committing suicide, and he may indeed go through with that.
PH represents the part of James that killed his wife, he's otherized his violent urges and frustrated libido. he wears the helmet because he's a prisoner. He wears the butcher's getup because he's a murderer. He has a gigantic phallic weapon because James is preoccupied with his frustrated libido and feels incredible guilt over the fact that he's thinking of sex while his wife is dying a terrible death.
