There are a couple of theories of what happened, the two theories I subscribe to is
1) Musk always used to be that deranged (as indicated by the pedo thing) but kept himself in check due to being afraid of getting banned from Twitter his autistic obsession so when he became the owner of the thing he became as unfiltered as he wanted. The other theory is that
2) Musk impulse bought Twitter (his autistic obsession) the same way you impulse buy something and later regret it coz it was a huge waste of money and in retrospect you didn't even need it/want it, only in Musk's case it was 44 billion dollars and loans with 3 billion interest payment each year. He regretted it because after signing the paper that said "I'll buy Twitter for 44bil" he tried to sue them to not buy it after all but he failed. As a result of the enormous debt he put himself in and watching his stupid monetization ideas throwing Twitter even deeper into the red than previously thought possible, he coped by taking even more drugs and getting even more addicted to Twitter posting, a combination of which turned him into a parody of himself.
