So the talent sends a request to Holo management, management forwards the request to Fromsoft, Fromsoft reviews it, approves or declines and then pings it back to holo management, who then indicate the answer and conditions to the talent.
The talent needs to provide a specific date and time for the stream in each case and since Hololive is obviously not Fromsoft's priority you need to allow for a pretty significant response delay along the whole broken telephone line.
From what's been said, Fromsoft perms never 'formally' lapse, they just stop responding to hololive emails outside of specific, arbitrary windows--typically the period surrounding a new release.
