On the other hand, your situation with that wall is simpler, you will just have to align the widget to his shoulders when you're done making it. I believe you could make a widget about this size (pic related) so you have more space to move around while aligning. When you crop his head with the smoke, you remove the inner part of the smoke and save the image as png so there's a hole for the battery bar to show. After that, it's all on UCCW. I believe the neatest way to set the bar would be vertically, in way that the battery lowers down (like the cigarettes smoke is lowering).
One more thing, after taking the screenshot for the cropping part, you will have to take the smoke off from the original image you're using as the wall. I just noticed it, but if you don't remove it and set the widget correctly, the battery will lower and the original smoke behind it will show, which "kinda" kills the hole point of this.
