The top module with the twink text and weather and clock is either UCCW or Zooper, could be some other random clock widget thing, but Zooper/UCCW will do the same shit. Fuck UCCW, go buy Zooper, swallow your pride and pay 3 bucks or whatever it is for it, fucking DO IT then make a widget on your home screen, fuck around with the preset widgets.
The icons are just custom icons, fucking simple. Make sure you're using Nova or Apex which you should already be using unless you're completed retarded, long-press on an icon and it lets you edit it.
For the specifics like the icons he used or the widgets, I don't know but just dick around on your phone - a nice clock widget, custom icons and an edited wallpaper are all you need to impress these shitheads
