It gets cluttered and personalised over time; and then when its getting too cluttered to be of any use, you give it a little cleaning. Clutter is usually more convenient than squeaky clean.

I think clean is best. Although, having a widget, launcher, etc is necessary, otherwise you're handicapping yourself.

Nice and clean all the time, with only the recycling bin and temporary folders. I don't even have the taskbar showing unless I scroll over it.

I rarely use the desktop since most of my work is either on one of the icons i use in the taskbar or i just [win + search] or ctrl+e to find the file on my starred. Would rather have more aesthetic for the desktop

The virtual desktop, like one's actual desktop, should reflect what you're doing there. When I'm two and a half months into a project my desktop is a god damned disaster. The day after I finish it tidies up nicely. Files and papers are deleted or archived and it's ready to be messed up again.
There's no sense in keeping it pretty all the time. Buy a painting.

Download Fences from Steam. It allows you to out icons in boxes on your desktop. Looks nice. On top of that you can double click the desktop to instantly hide all icons. So it can basically be cluttered and clean simultaneously.

They can be both clean and personalized. On windows 10 I have a black background with no icons, hidden bottom bar unless you move your mouse down, and launch everything from start menu by pressing win key. I also boot into linux with a tiling wm because of more freedom to personalize and extra comfiness. You can do more subtle changes and tailor the setup to be your own system (tm) by playing with the color schemes, writing small scripts and assigning key bindings to them etc.

You have to edit some config files to make the bar transparent. It's not a special software, but it's not an "easy toggle" function either
