We bought a JR Pass and moved around the country after a pretty short time in each town, we mainly visited pretty popular places, actually asking google "things to do in X" tends to yield good results
There are beautiful things everywhere, so no matter where you choose to go, you'll probably be pretty happy.
However I'd recommend staying in another city than Tokyo, Hiroshima is awesome and has tons of beautiful sights around, Kyoto is also beautiful but crowded during holiday periods
Compared to my country (France), hotels and restaurants are way cheaper in Japan, so we were able to stay in good hotels during the two weeks, even splurge a bit on renting a traditional house for a couple of nights
10 US bucks will get you a good meal, double it and you got a big good meal and probably a drink of your choice

Guesthouses aren'd bad either usually, so you can stay in town for very cheap, less than 30 dollars a night last time I checked
Tattoos are mainly prohibited in some bathhouses, usually smaller ones, the big ones in Dogo Onsen, or in big hotels that have them don't have a ban.
I haven't personnally encountered a "no tattoos" sign yet, and I do love me some onsen time.
My personal hidden gem is Hiroshima Mitaki-Dera, a buddhist temple complex on the outskirt of Hiroshima. You can easily get there with a bus, indicated on google maps, which can take you straight there.
It was the first place of the sort I ever visited, and it was simply magical; please go there if you decide to go to Hiroshima.
Miyajima is wonderful, but there can be a lot of people, though I also highly recommend.
Fun getaway from Hiroshima, which I also recommend: take a ferry to Matsuyama, and stay around Dogo Onsen!
