How do you go about creating a game with an optional content patch in mind that the user can install themselves? Check if file exists, and don't do anything if it doesn't exist, or something else?

its called modular design and no they aint doing these bc dem gacha for more money
not to mention that often times free dlc is frowned on as studio aint making more money on them for people with no sense of gratitude is running or deciding for studios, maybe if you dont care much for profitability or selling like hotcakes where cost of a dlc isnt much to ask

Your middleware layer should surely handle this for you, no?

If not, you need to write your own file-opening code, that handles multiple sources. Assuming your game's not fuckhuge, you can just use something like WAD, ZIP or TAR, and when the game launches:
- read the filename, compression type, and offset into a dictionary
- repeat for each source file, overwriting entries that are already there
- sort and index ready for your game's VFS layer

Keep in mind that you don't have to load this shit from the game directory. You can scan the saved game directory, load any content bundles you see there, optionally check their signature, then add their blobs to the to the object-paths the games sees just as if they were installed.

This way you get 100% plausible deniability WRT family-friendly distributors, because the "additional content" isn't part of the game and doesn't go anywhere near the files the distributor manages. It's just a file the end-user got from a third-party and put in their saved-game directory.
