That one is applied externally to the female genitals and held in place with adhesive tape though some electrodes are intended for males and some are meant to go on the ankles so the current passes through ankle to pelvis then out other ankle.
Electrostimulation has been around for over a century. Unlike vibrators which were also medical inventions this one hasn't caught on so much with the public aside from those useless "no exercise muscle contractors/stimulators". Possibly this is because vibrators are easy to make, even bad ones are pretty safe, but a poorly made electrical stimulator presents a painful and possibly harmful risk. Good ones exist but you're looking at hundreds. The self adhesive electrode pads usually aren't reusable, maybe use tape, and the internal probes need cleaning but so do dildos so that's not really a big deal.
I've never heard of a device that can be used by a couple for penile/testicular and vaginal/clitoral stimulation at the same time. The constantly changing conductance with change in electrical pathway as the penis enters and exits would have to have the device either be set to a value safe for maximum entry which would mean lower sensation when partly withdrawn, or would need real time current and voltage sensing to dynamically update the stimulation and prevent unwanted changes in current that could prove shocking.
