Was it the same number of panels? Could it have been different image dimensions? How long ago did you see it on /a/? I've tried a few different ways of looking for it and I haven't found it. I put the 4chan one through yandex and tineye, I put other variants through, I whited out everything but the lines and the faces (not quite making a clean template because my method required changing the dimensions and lower res, don't ask me why), put that through them, got nothing. I put the dimensions of various variants (500x1200, 501x1200, 500x1201) in the 4chan archives for numerious boards, and tried a number of searches, including "nope perfect", which netted me results, but no blank template.
