Her name is Akechi Kokoro, and she's from the anime, game, novel, and radio drama "Tantei Opera Milky Holmes." From what I gather, it's a detective story where people are given superpowers by Toys. The fan-made wiki for the franchise didn't have spoilers on anything that happens to her, but TV Tropes doesn't have a horrific death trope, or really anything dark at all, on their page for the series
As far as I can tell, she's still alive and still annoying the rest of the cast
