It was about a boy and his disabled sister going to elementary school. The girl had some disease that left her looking like Stephen Hawking without a wheelchair and she required one of those medical nose tubes.

It looked pretty old, possibly pre-80's. I knew what the ending was probably gonna be so I skimmed through it until the last 40 mins where it had me crying anyway
