Murcielago seems to gets pretty gruesome, but I only just started reading it myself, so I can't say how far it goes.
Many Junji Ito works have their fare share of gore, but he tends to lead into distortion and body horror than blood and guts.
Franken Fran is a personal favorite of mine.
The Corpse Party anime seem to go pretty far with their gore, but once again, haven't seen it for myself.
