Man, Homunculus was so good.
I don't know if those fit exactly, but when it comes to manga I can definitely recommend Himizu and Freesia. Both are very uncomfortable to read, have mentally unstable protagonists and are pretty psychological anf messed up in general.

Fourteen by Umezu has a lot of camp to it, but it really does read like something written by a genuinely mentally ill person. The story is ridiculous, but with each chapter things grow more and more oppressive and disturbing.
