I really don't get how people mess up making burgers. It's the most basic thing, and what makes a good burger is so well established that you simply can't not know it, if your occupation is something like a chef.
So basically, in practice the only way possible that you could make a burger bad, is if you were more bent on being quirky and different than making something that's actually enjoyable. Hope you didn't head over to that restaurant again, because that's the worst kind of one that you could go to. There's just no need for stupid experimentation with something like a burger, you know? Just make it good, goddamn.
But I can't comprehend how anybody would consider it a good idea to place a full disk of onion in a burger anyhow. Whether they were trying to be different or not, that's just placing something blatantly terrible in there, how would you ever consider it a good idea?
I feel like those kinds of chefs haven't even ever ate the things that they serve people, which might sound like a far-fetched accusation, but it's something that's very apparent with some other professions already. And with how some foods are, I really can't think of any other reason as to why it would be served to anybody.

You know what they should make, Undercover Boss with restaurants, but instead of the owner of the company paying attention to ethics or whatever, they just look at the kind of the food that gets made, and if anybody gets caught doing something like placing a full disk of onion in a burger, they get burnt alive on live television. Oh, wait, that's kind of just what Kitchen Nightmares is. But, they don't burn those people alive.
