His obsession and lack of social skills are the defining factors in his "Otaku-ness" not whether or not he runs away from reality.
We call someone with an obsessive fandom, and a severe lack of social skills "Otaku". We call someone who is running away from Reality "Psychotic" or "Delusional", depending on what the source of that inability is. Fun Fact: "Fan" is etymologically derived from "Fanatic", which was a 17th Century word used to describe zealots. Take it even further back, to it's latin base, "Fanaticus" and you have the definition of "mad, insane, inspired by god/s". If it hadn't morphed into such an acceptable term to use to describe oneself today, people would shudder to use it as a self-descriptor.
