NGE's canon is loose enough for it to just be the appearance of Rei that would be weird.
Since there isn't any familial bond or relation between the two besides their genetics, it's like hooking up with a half-sister that got lost at birth who you later reunited with. It'd still be incest since there's a level of similar DNA, but none of you'd know nor make any of the associations.
Shinji ends up knowing afterwards though.
Worst case, Rei has a 100% set of similar DNA to Yui (his mother), but this isn't likely TBH. It could also be likely and possible the percentage is 0% given the canon you pick out.
Sadamoto has said it was 50/50 at one point.
