Although the differences might seem subtle, there's a huge difference in how sexuality, prostitution, and Roanapur in general are portrayed between them. Murder twins aside the manga is very sex positive, especially regarding "unusual" relationships. Garcia expressing sexual desire for Roberta is shown as healthy and unobjectionable. They get to have a happy ending, running off into the sunset together. Revy's time as an S&M dancer is a positive symbol of her sexual empowerment. Her problems stem from the world discriminating against her for expressing her sexuality. Prostitution is voluntary and consensual, with pimps there to provide protection to the girls. Roanapur is a great place, everyone should want to live there.
The anime takes a darker, sex negative view. Garcia's desire for Roberta is unhealthy and sick, a mother/son relationship gone horribly wrong. Roberta is severely wounded in her final encounter with Delta force, is disgusted by Garcia's advances, and runs off to join a nunnery. Revy's rape as a child defines her character, leaving her avoidant and fearful of intimacy and sex. Her problems don't stem from any choices she made, just the world deciding to fuck her over. Prostitution consists of pimps beating the girls until they'll accept anything from the Johns. Roanapur is a horrible place, and everyone should feel some sympathy with Rock when he tries to have the whole place burnt down. When you watch the anime, you get a whole different view on thing than when you read the manga.
Although most people don't consciously realize this, it leads to things like anime fans feeling like Revy seeing her time as stripper positively as weird and OOC and manga fans viewing Revy's rape as unnecessarily dark and out of character with the rest of the series.
