It shows she's willing to fight, and not necessarily frail, but you can't deny that this girl on the stretcher is in pain and is in no shape to actually fight.
It's a mix of both which does imply strength, but because of her injuries you as the third person should feel obligated to fight in her place so that she won't have to suffer.
It does have elements of being a damsel in the sense that she's in distress (pain, injury), but she'll still fight.
I see both your points but Rei's frailty synergizes with the strength she shows, so that Rei is the savior and strong one. Protecting Shinji, opting to take the most dangerous positions, and of course that tendency towards self-sacrifice make that clear to me.
