I have a friend who did the transition when she was 21 or 22 and was masculine and looks ok with only hormones and laser bear removal. She's talking about going to a doctor for facial surgery, so I think you can do stuff about the bones too! People will react weird no matter what you do and most eventually come around. Your brother might secretly be your biggest supporter if you do it! Talk to people who did it (check /lgbt/ maybe?) and maybe it would be a cool thing for you to do, or maybe not.
