But I think Hanako isn't autistic enough not to understand that her relationship with Lilly (and, later, with Hisao) is very very similar to a codependency one. Lilly is certainly sincere in her actions and in her desire to be friends with Hanako, but honestly, I think she does this to feel pleased with herself: she likes to treat Hanako like a child (like she does with anyone) because it's her "motherly" way to deal with other people, and she probably feels good acting this way. Lilly's kindness is what lead them to became friends; but, after some time, this kind of mother/retarded daughter relationship began to hurt Hanako, who started to consider herself even more useless and "broken", as she says.
Lilly tells Hisao more than once that Hanako is not as fragile as one might think, but she does nothing DIRECTLY to prove it (or to stimulate Hanako's independence): Lilly advises Hisao to give Hanako more space because that's what she doesn't do first!
So yeah, obviously Hanako thinks of Lilly as her best friend, but, without a doubt, theirs is a complex and not always healthy (for both sides) relationship. And Hanako knows that
