I know that probably isn't true anymore and many people, especially younger kids, probably first see her in some Smash Bros. game nowadays. But let's ignore that. In OoT, you are introduced to her for the first time in the Temple of Time, right when Link ages seven years. Here is the cutscene for reference:

Then it cuts away and you can see her standing behind Link. Her face is not in the shot. It is also a pretty brief. I think most people would notice how strange her clothes are and the weird symbol on her chest. When they finally show my Sheik's entire body, I'm sure all of this culminates together into them wondering, who is this? Why does she wear such strange clothes? Although... I always hate to say this, but many wouldn't be thinking "she", they would be thinking "he" until they know better.
I first played OoT when I was very young, so I can't really say what my very first impression was. I can't remember. All I do remember is that I liked the character as a kid. I find it really unfortunate that I can't remember my first time seeing Sheik. I'm sure many anons on here do remember their first time seeing their wife. It's not fair! I want to remember my first time seeing my Sheik! I guess that's what happens when it takes so long for someone to not only fall in love, but admit their feelings to themselves. Plus it seems like my Sheik has some of the oldest source material out of everyone else's here, so something like this was bound to happen. It just sucks is all.
