Some quick answers though:
I use the path tool in Gimp, the Draw Path dialog there allows sharp endpoints for hair strands (probably also possible in PS too).
The outline is 5px wide usually while rest of the lineart is 3px wide, which is enough to cover the original lineart (assuming 1920x1080 resolution).
Images are resized to around 500px after everything else is finished.
