Sheik's perception her mind grows up as well?I will answer this question, but I would like to explain how I think Sheik's mental age works before I describe Zelda's. I think Sheik was created with a certain age/maturity in mind. Zelda was 9 when she first transformed into Sheik. So she was in a child's body initially, not in an adult's body. I believe the triforce would have made Sheik with that age/maturity in mind, but her experiences changed her into a much more mature girl. Let's not forget that the difference in maturity between 9 and 16 is huge. Adding the experiences she had on top of that, plus probably some influence from the triforce, makes her mental age seem much older.
While our little theory about Zelda being able to interact with Sheik is nice, it is more than likely that Zelda is sealed, like you said. I believe that when Sheik transforms back into Zelda, she gets a wave of memories and she also understands it near immediately. Zelda herself is now forced to remember traumatic events and witness what her creation went through. Even if the memories are nearly instant, I believe in her mind, it would be a long moment of remembering. It is almost like Zelda is experiencing trauma in that very moment. I believe it ages her mind pretty quickly. Maybe it takes more time for every change to happen, but there is an immediate affect. Unlike Link though, there are actual memories and experiences, so I don't think it is a child living in an adult body.

Sheik would look entirely Sheikah and any physical characteristics she shares with Zelda is a coincidence. Sheik would NOT look how she does now. Now, I don't know what she would look like, but the Sheik I post and the Sheik that would exist in a separate body are two completely different looking people.
