Only longstanding ongoing conflicts (where no party to the conflict had nukes at the start of the war, such as the many Indo-Pakistani conflicts), undeclared border skirmishes (like the Sino-Soviet one after the split), and covert actions where the government officially denied any involvement (pretty much the entire Cold War).
