I thought that maybe if there was a chip in the wheel it could rip the tube.If there's a burr on the rim it could easily damage a tube, but you can easily check for those just by feeling. Also check that the rim tape/strip is fully covering all the spoke holes and isn't torn at all. If it happens again pay close attention to where the hole in the tube is in relation to the wheel and examine the wheel and tire near that spot extremely closely.
