No, that's the Brit norm, because you can use the rear brake in an emergency more safely than the front one.
It's done like that so the arm that's on the inside of the road can move more freely to indicate, that way you won't OTB if something startles you and grab the brake too hard.

No, the toe-in is very slight, so that when you brake the whole pad still makes contact
Think of it as walking, your heel makes contact first but your whole foot quickly follows, your heel is not wearing out faster

The american way is objectively correct, because the centering screw of each brake caliper is on the same side as the corresponding lever, so you can open and close the brakes and adjust the screw from one side without having to reach across to the other side of the bike.
