Some dipshit tossed a cigarette bud down a vent shaft and set a massive fire to the aircraft carrier, enough that it had to cancel it's deployment and go back to the states for repairs.
Now whether the captain smoked or not, his ass was on the chopping block, along with the XO and a few others.
By the way this shit isn't limited to the Navy either. I've got a chairforce buddy of mine who told me about them accidentally putting nukes on a plane sometime around 2007(?) and not finding out about it until the plane landed. On one hand, how the fuck do you lose nuclear weapons like that? But on the other hand it might've been a tad excessive to fire and kick out everyone from the secretary on down involved with it. Talk about taking a meat cleaver to the chain of command, I bet the blood was knee deep by the time they were through.
