It has the shoes, pin covers for the shoulders, a rifle (not the one pictured, but it's also toy-inspired), the pointy "handle" part, and a chain for the Matrix. Highly recommended, the "slippers" are great and don't hinder transformation, but you do have to take them off to adjust the heels. It also includes a sheet of decals for a few spots but I chose to paint those instead.
