Coat is from a Destiny Warlock
Torso is from a Heavy Borgia guard (the blue and gold one)
Upper torso (where the extra arms attach) is from the blue Destiny Fallen Captain from the Sparrow set.
Arms are from He-Man, obviously
Head is an elf from the WoW line
Harness is from a movie TMNT Raphael, but I think it's backwards. It gives the two torsos some extra stability, but they stay together fine on their own.
