The Pokemon comes out of the Pokeball when said Pokeball is broken. They can also come out of the ball at will, so if you lost it somewhere, that Pokemon would eventually come out, like someone who's been in a hide and seek game for too long, wondering where you are.
Kinda makes me want to write a story about some loyal Pokemon that gets seperated and seeks out thier trainer across regions.
