Like, why would he care about what the neighbors think of his screaming? He's severely autistic, so the thought likely never crossed his mind, consciously or not. But Leaf is presumably not autistic, so she is aware of the kind of social expectations that make that unacceptable behavior.
Also the entire scene at the pharmacy sounds like it was taken verbatim from real life. No fucking way did he make that up.
