I remember hearing about one serial rapist who always gave his victims oral because he genuinely believed they enjoyed it and wanted to be raped. He was pretty obviously mentally ill, to the point that after realizing what he did was wrong, he said he never wanted to leave prison because he was 100% sure he'd do it again anyway.
