I remember hating it coming off of HGSS but looking back now I have nothing but nostalgia.

What changes/additions would you do to make it the best pokemon game it could be? (Hardmode: without adding pokemon from old gens)

make an actual postgame that isn't just walking through the 4 routes the game roadblocked for no reason
put autorun and following pokemon and a usable bottom screen back in
make the difficulty not piss easy garbage where the gyms only have 3 pokemon that they forget to give EVs

That would make it worse anyway. The best thing about BW was that Game Freak creating a massive new roster and actually fully utilizing them. They learned their lesson from HGSS and saw how despised Johto Pokemon were because Game Freak stupidly created only 100 pokemon, made way too many relations to old Pokemon, and flooded the region with KANTOOOOOOOOO Pokemon. Game Freak wanted the dex of Masuda’s Magnum Opus to avoid being forgotten forever and seen as nothing more than Kanto’s Expansion pack.

Give the early route trainers more diverse teams holy fucking shit fighting the same purrloin, patrat and lillipup gets stale really quick, also if possible, make the rivals battle you less often, I want to explore the new routes rather than have to run crying to the pokemon center because cheren wants to beat my ass and take my lunch money since I dared to get a gym badge

Add back all the content it removed from gen 4 like battle frontier, contests, safari zones, secret bases, underground, partner follow, events for mythicals, shiny huntable legendaries, vs seeker, poketech, capsule balls and non-conventionally obtained raremons (Munchlax, Feebas, Spiritomb) and this could easily be the best pokemon game. Which is why a Hoenn remake in gen 5, and a Grey version that inherits features from said remake, would have been amazing.
